Before go to the online form please read instruction carefully that your application is meeting the eligibility criteria (Fee once paid will not be refunded)
1-Click on Memberships -> Online Form -> Member sign up and complete the application form and create password with your e-mail id
2-Go for member’s login with your E mail id and Password
3-Complete your form .
4-Uplode your photo in JPG format not more than 60 kb
5- Upload your documents in PDF format (all the documents 10th & 12th separately and then upload: Diploma in Optometry/ Degree in Optometry must be compressed in single copy) not more than 2 MB.
6-uplode proof of Payment in JPG format
7-Before final submission Please check your form if all is ok Please go for final submission
8-Take a print of your Form sign it and send with self attested photo Copy of your Documents ( mark sheet/Certificate of 10th,12th (Science),Diploma in Optometry/ Degree in Optometry) to the Mailing address Visual Aids Centre, 8 – Ring Road, Lajpat Nagar – IV, New Delhi-110024
9- E-mail the signed form to membershipioa@gmail.com with a copy to the President at – ioapresidentsk@gmail.com and to the Secretary at – rajeevioa37@gmail.com
